
After intake, the youth is offered a case manager for follow-up services with his or her family. The case manager reviews progress with the family every 7, 30, 60 and 90 days to assess their needs. Case managers are also available throughout the 90-day enrollment period for any requests from the family for further service referrals or crisis intervention. At discharge, 90 days after the youth’s enrollment in services, the case manager, youth and family assess the youth's progress to determine the effectiveness of community-based referrals. Discharge recommendations are made to the family to help keep the youth from re-offending in the future.
