Montgomery County - Wraparound in IFPS

Maryland Choices utilizes a process called wraparound: an approach of coordinating highly individualized, family-centered plans of care that focus on the unique needs and strengths of each family. Supported by a care coordinator, families work as drivers of their own care and interact directly with providers serving them. Maryland Choices’ care coordinators act as facilitators and strategic interventionists for the family and guide them through the process of child and family team meetings.
