Maryland Choices - Group Home Diversion

DJS Group Home Diversion Program and DHR Group Home Diversion Program are available to youth in the custody of DJS and DHR, respectively, who are at risk of a group home placement or are stepping down from a group home to a less restrictive environment such as a relative’s home. Youth are served through a wraparound process, with a family-centered and individualized plan of care.

To make a referral, or to make a self-referral, please contact Maryland Choices. You will be asked to complete a short referral form and may need to provide other documents such as assessments, intake summaries, or other historical documents. Youth and families that are a good fit for this program are:

• willing to stay in the home placement
• available for intensive, in-home and community-based approaches to care and are
• prepared to be part of the child and family team.
