
Care coordination is the process by which supports are linked back to the youth based on the plan created by the child and family team. Care coordinators work in each youth's community to link them to natural supports and services with the ultimate goal of completing the goals of the child and family team and transitioning to the least restrictive environment possible.
The care coordinator helps monitor the implementation of the family's plan of care. They ensure all of the necessary supports are in place and in line with the plan, and make all other individuals involved aware of the family's unique needs and strengths.
