Child and Family Teams

When a youth is referred to the Maryland Choices, a child and family team is formed. The goal of this team is to prepare the family to become the coordinator of supports the youth needs to succeed in the community. Goals are defined by the family and the team members work together to achieve those goals.

A child and family team typically consists of five to nine members, including:

Youth and family members
Care coordinator
Community members
