Maryland Choices - Becoming a Provider

Becoming a provider is a quick and easy process. We are happy to now provide many of the necessary resources online to help you get started.

Step 1: Download the provider profile and submit it to your region's contact at the right.

Step 2: We will review the profile and schedule a Maryland Choices orientation and provider presentation of services.

Step 3: Upon agreement to enter into a contract, complete the provider agreement package which will be provided to you. Submit it to your region's office.

Step 4: Maryland Choices reviews the provider agreement and application. Upon approval, you will receive service codes, contracted rates, invoicing instructions and effective date.
