Community Members

Community members may include representatives from referring agencies, friends and neighbors, faith-based members, and providers. They work with the team as a whole to help the youth and family set and reach goals. Team meetings take place at least every 30 days.

Community team members:

• Assist in the development of the care plan
• Offer information to the team that helps guide the plan
• Participate in the approval of the care plan
• Participate in the care coordination team
• Implement the care plan and share successes
