Becoming a Provider

Becoming a provider is a quick and easy process. We are happy to now provide many of the necessary resources online to help you get started.

Step 1: Review the provider memo and complete the provider questionnaire. Submit the requested materials to Sharidon Wolz. For assistance, call 513.765.5503.

Step 2: We will review the questionnaire. If your services match our existing needs, we will schedule a Hamilton Choices orientation and provider presentation of services.

Step 3: Upon agreement to enter into a contract, download and submit the provider application. A provider agreement will be provided to you.

Step 4: Hamilton Choices reviews the provider agreement and application. Upon approval, you will receive service codes, contracted rates, invoicing instructions and effective date.
