What Makes Us Different

Hamilton Choices utilizes a funding structure and case rate that allows it to coordinate a wide array of support for youth, from highly intensive services such as respite care or foster care, through community-based and natural supports. Combined with highly individualized care planning that concentrates on the unique attributes of each youth, Hamilton Choices has become highly successful at helping reduce lengths of stay in residential treatment, while supporting youth to keep them with their families or in home-like environments.
