DC Choices - School Wrap Care Coordinator

The School Wrap Care Coordinator is responsible for completing a comprehensive strengths-based assessment of the student, working in full partnership with family, school and community teams members to develop a plan of care, on educational needs, behavioral and community needs. The School Wrap Coordinator oversees the implementation of the plan, identifying providers in-school, community and family resources, facilitates monthly school team meetings; makes regular classroom and home visits, monitors all services authorized for student care and authorizes all care to main fiscal accountability. The School Wrap Care Coordinator assures care is delivered in a manner consistent with strength-based, family centered, and culturally competent values, offers consultation and education to all providers regarding the values of the model, monitors progress toward treatment goals, and assures that all necessary data for evaluation is gathered and recorded.

Qualifications include:

• Minimum of Bachelor's degree in social work or related human service field. Master’s degree in social work or related human service field preferred.
• Three years of clinical/management experience in human services field preferred.
• Demonstrated two or more years of clinical intervention skills.
