The process of individualized treatment planning starts with the Child and Adolescent Needs and Strengths assessment (CANS), which serves as the primary clinical tool for Choices care management entities. The Dawn Project, Hamilton Choices and Maryland Choices rely on data to inform decisions at multiple levels. The CANS is a critical tool for these systems of care. At the clinical level, CANS data inform treatment planning, training and quality improvement efforts. At the management level, the CANS helps make level of care and placement decisions through the use of locally developed algorithms. Finally, at the organizational level, the CANS is used to measure outcomes that influence funding and system-level decisions.
Choices Awarded for Outcomes
Choices, Inc. received the Outcomes Champion at the Agency Level award, presented by John Lyons, Ph. D. on behalf of the Buddin-Praed Foundation at the 4th Annual CANS Conference. We have been recognized for adopting the CANS as our primary clinical tool, using it as part of a larger Total Clinical Outcomes Management framework and fully integrating the tool into our practice in Indiana, Ohio and Maryland. The way in which Choices has embedded the CANS into its clinical management system was a key reason for receiving this award because of the role that TCM plays in helping us use the CANS for service planning, supervision and program evaluation.
