Make a Referral to Choices, Inc.

DCS Referrals

1. Complete a referral to Cross System Care Coordination in Wizard
   • Choose "New Referral"
   • "Family" - Identify family to be served
   • "Services for Children" - Choose SOC/Pilot Projects / CSCC / and:
     • Level 1 / Green
       For youth in early system involvement
     • Level 2 / Yellow
       For youth in foster care
     • Level 3 / Orange
       Undecided/Unknown, or for youth in therapeutic foster care, group home or supported independent living
     • Level 4 / Red
       For youth in residential treatment
2. Download and complete the CSCC Referral Form, Authorization for Release and Exchange of Information, and Consent to Treat.
3. Submit these, along with your most recent CANS and any supporting documents, via email or fax to 317.202.4203.
4. Download these documents and keep them for your records.

Probation Referrals

Using Quest

1. Complete your referral using Quest
2. Grant Choices program access for the youth you wish to refer
3. Submit the following information via email:
   • Youth's name
   • Quest file number
   • Link to approved referral form

Referrals without Quest

For offices not using Quest, please use your local referral process and email us to make a referral.
