Individualized Care
At Choices, we have taken the idea that supports and services should serve the unique needs and strengths of each individual served and their family, and expanded it into a model where those same individuals are also driving their care. We take pride in the fact that youth and families are the ones who tell us what their needs and strengths are and what supports can meet those needs. They drive planning processes from the very beginning and are equals with our team members in creating, implementing and assessing the success of their plans.
With youth and families in the driver’s seat, we can assure that we’re not just providing individualized care, we are providing a venue for families to grow and become their own coordinators of care.
By giving families the freedom to drive their care, they determine plans of care that best represent their culture, ultimately leading to care that is culturally competent. This is in contrast to individualized care where professionals determined what the family needs which may not be truly representative of the family’s unique culture.
